All 4 Ancient Earrings + x4 Accessory Medals ()= Sapphire Earring 

Necklace:
All 3 Ancient Necklaces + x4 Accessory Medals ()= Sapphire Necklace

Ancient Belt + x15 Accessory Medals ()= Sapphire Necklace

Accessory Heart () can be obtained from the Boss of the Hidden and Normal Temple of The Exile dungeon.
Ancient Dark Magic Stone () can be obtained from the Chamber of Heroes dungeon. (The daily dungeon)
